At the beginning of decay, only the top layer of enamel of the tooth is reached. At this stage, the patient has no pain. This is the best time to treat caries.
When decay reaches the dentin, the tooth is sensitive to hot and cold. The patient may feel pain, but it is not persistent. At this stage it is still possible to settle the issue through restorations only.
When decay reaches the pulp cavity, the patient feels a sharp pain and persistent. In addition to major restorations, root canal treatment is unavoidable.
